Welcome to the Indiana State Department of Health (ISDH) Health Care Facility Information Center.  The Health Care Facility Information Center is part of the ISDH Health Care Quality and Regulatory Commission.  The Commission serves as the survey agency for state licensing and federal certification of health care facilities.  The Indiana Health Care Facility Information Center is intended to provide healthcare providers with easy access to program policies pertaining to the licensing and certification of health care facilities.

Program Advisory Letters

The ISDH periodically issues Program Advisories to provide information to health care facilities about program policies and procedures concerning the licensing of health care facilities, agencies, and centers. This Health Care Facility Information Center is intended to serve as a clearinghouse for all Program Advisory Letters.
