All test dates are on a Monday. Testing begins at 10:00 a.m. at the Indiana State Department of Health in the Indiana Government Center North Building, 100 N. Senate Ave., 8th Floor (N855),  Indianapolis, IN 46204, unless otherwise noted.  Please take WEST elevators to the 8th floor.  Contact the Indiana Lead & Healthy Homes Program at 317.234.4423 to make arrangements PRIOR to taking the 3rd party exam. 

You will need to pass through a police-manned metal detector to enter the Government Center building.  Please leave any item in you car that will not be allowed in the building (i.e. pocket knife) prior to arriving for the 3rd party exam.

Arrangements can be made to take the exam in Allen, Elkhart, Lake St Joseph or Vanderburgh Counties.
