On January 11, 2005, Gov. Mitch Daniels issued Executive Order 05-10, requiring the Indiana State Department of Health to develop and implement a medical error reporting system (MERS).  The purpose of MERS is to reduce the frequency of medical errors, reveal the causes, and empower health care professionals to develop methods to prevent errors or discover errors before patients are harmed.
