I'm looking at Table 5 in the Indiana Mortality Report. What do the various columns mean?

There may be some confusion when looking at the various tables in the Indiana Mortality Report. Tables 1 and 5 have the same set up. Some people have questions about what is contained in the columns. Some people have added up the total number of deaths in White, Black, Other and Hispanic and it doesn't equal the total number of deaths listed. Some questions have been asked about the causes of death - how do they know if one cause of death is a subset of another? What are those codes? The example provided in the next section will explain the various columns in Table 5, which will also work with the other tables in the report.
