The Indiana ASIST 2010 Project is a 3 year project running from September 1, 2007 through August 31, 2010 with federal funding from Health and Human Services (HHS). This project operates under the Office of Women’s Health at the Indiana State Department of Health. Indiana ASIST 2010 will focus on Healthy People 2010 objectives for Diabetes and the cross cutting focus areas of Nutrition and Overweight and Physical Activity and Fitness. The Indiana Diabetes Advisory Council will serve as the public health system/collaborative partnership for this project. A gender focus will be achieved by creating a Women’s Health Committee (see attached roster) under the Indiana Diabetes Advisory Council that will also serve as a steering committee for the grant. The Office of Women’s Health will hire a full-time project director to serve as the committee chair and oversee all aspects of the project. A part-time epidemiologist (Carolyn Muegge, Bowen Research Center) will be contracted to assist with surveillance tracking, data collection, and reporting. Three community partners (northern: Elkhart County Health Department, central: Partners for a Healthier Community {Howard County}, and southern: Hoosier Uplands {Lawrence County}) will implement a pilot patient navigation system for female diabetes patients. Each navigation system will utilize evidence-based strategies to focus on specified Healthy People 2010 objectives (see below) and improve outcomes for Hoosier women with diabetes

· Diagnosed by a physician with Type 1 or Type 2 Diabetes

Diabetes
5-1 Increase the proportion of persons with diabetes who receive formal diabetes education.
5-12 Increase the proportion of adults with diabetes who have a glycosylated hemoglobin measurement at least once a year.
5-13 Increase the proportion of adults with diabetes who have an annual dilated eye examination.
5-14 Increase the proportion of adults with diabetes who have at least an annual foot examination.
5-15 Increase the proportion of adults with diabetes who have at least an annual dental examination.
5-17 Increase the proportion of adults with diabetes who perform self-blood-glucose-monitoring at least once daily.
Weight Status and Growth (need height and weight to calculate BMI)
19-1 Increase the proportion of adults who are at a healthy weight.
19-2 Reduce the proportion of adults who are obese.
Physical Activity in Adults
22-1 Reduce the proportion of adults who engage in no leisure-time physical activity.
