If you have a question or concern about HIPAA’s application to one of the following programs, please contact the Office of Technology and Compliance at the number listed above:

For HIPAA concerns or questions regarding non-State Department of Health entities, please contact the entity and ask to speak to their own privacy officer and/or contact the U.S. Department of Health and Human Services Office of Civil Rights, and/or the Indiana State Attorney General's Office via the link below.

Filing Complaints

Please note: Complaints about HIPAA violations at a non-State Department of Health entity should be addressed to the entity in question and/or the federal Office for Civil Rights and the Indiana Attorney General’s Office as the Indiana State Department of Health has no power to enforce HIPAA.

Any concerned individual has the right to file a complaint concerning any suspected violation of the HIPAA regulations, even if their own health information was not involved. If you would like to file a complaint about one of the State Department of Health’s HIPAA covered programs, please see below.

  • Click here for the ISDH HIPAA Compliant Form. The Office of Technology and Compliance would appreciate an opportunity to address any violations internally prior to the Office for Civil Rights involved; however this is not a requirement.
