ISDH Health Care Facility Consumer Reports

In 2003 the ISDH created its first health care facility consumer report.  That report was for nursing homes.  The ISDH has since added additional consumer reports for other provider types.  Each consumer report provides information about health care facilities and entities.  The information includes location, contact information, administrators, services provided, and survey history.  Consumer reports are now available for:

  • Ambulatory Surgery Centers
  • End Stage Renal Disease (ESRD) Clinics [dialysis clinics]
  • Hospice Agencies
  • Hospitals
  • Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF-IID; formerly ICF-MR; also called group homes)
  • Nursing Homes (Comprehensive Care Facilities)
  • Residential Care Facilities (Licensed Assisted Living Facilities)

Centers for Medicare and Medicaid Services Guides

The Centers for Medicare and Medicaid Services (CMS) has created several comparative guides to assist consumers in identifying and evaluating health care facilities and providers.  The following are links to the CMS Guides:
