Although it's best if you can enter information directly into HMIS, we know that it's not always possible. If your internet access goes down, or if your HMIS staff are not working then you can use these paper forms to collect all the necessary information. Please make sure that you are not giving these documents to your clients to fill out. Make sure that the person filling out this form has been trained and understands all the definitions. Refer back to the Intake Definitions documents in the Training Material section for more instructions.
HMIS Intake Form - updated 6/3/10
This form captures all of the required demographic information about the client when they initially enter the program.
HMIS Discharge Form - updated 6/17/10
This form captures all of the required demographic information about the client when they are discharged from the program.
Household Form for Other Adults - updated 6/9/10
This form captures information about the other adults in the client's household. Print out one form for each additinal adult in the household.
Household Form for Children - updated 6/9/10
This form captures information about the children in the client's household. Print out one form for each child in the household.
HMIS Annual Updates Form - updated 6/9/10.
This form captures all of the information that is required to be updated at least annually for every client in a homeless program. These updates are entered on the Face Sheet in the HMIS Updates section and on the Household Form in the Updates section.
HMIS Supportive Services Checklist (Contact Logs) - updated 5/19/10
This form captures information about the services that are provided to the client during their stay in the program. This version of the Supportive Services Checklist is for homeless programs. HPRP programs should refer to the HPRP Training Material as they use a different version of the Supportive Services Checklist.
