Registered Agents/Service of Process

A Registered Agent is a company or individual designated to receive legal notice when a client is involved with legal issues such as a lawsuit or summons.  The Commissioner is NOT the registered agent for all insurance companies.  Indiana utilizes the NAIC Uniform Consent for Service of Process (Form 12). Any updates to registered agent will need to be submitted electronically utilizing the NAIC UCAA Electronic Application System.  This can be accessed via the below link, once there you will click on the Industry link at the top of the page and link to the electronic application is located under Uniform Certificate of Authority section.
