The following state specific items must be submitted along with the Uniform Certificate of Authority Expansion Application (UCAA) in order to obtain a Certificate of Authority.
- Articles of Incorporation approved by the Secretary of State of Indiana, in accordance with I.C. 23-1-3-1 et. seq.
- The IDOI utilizes the Uniform Certificate of Authority Expansion Application
