Maintaining Your Application
We have changed the name of the "Statement of No Change" form to the "Affidavit of Continuing Eligibility" or "ACE" form. As a reminder, although your certification is valid for a three-year period, you are required to submit this annual form with any changes affecting the following:
- Change in location or contact information (address, phone number, e-mail address, etc.)
- Change in services provided (amendment to certification)
- Change in ownership
- Change in control
In accordance with our mission to be customer focused, the Division will continue to accept the old forms.
