If a facility is closing, facility representatives are required to submit a Notice of Termination/Exclusion (NOT/E) Letter (State Form 54087 available on the IDEM Forms page) to IDEM. Once approved, the submittal of a NOT will terminate permit coverage for the facility under Rule 6. Note: all industrial activities and materials must be ended or removed from contact with storm water run-off prior to the NOT/E submittal.
An NOT request due to closure must be submitted within 30 days of the date of closure.
