Introduction

The Hazardous Waste Biennial report is required by 329 IAC 3.1-7-14, 329 IAC 3.1-9-1, and 329 IAC 3.1-10-1. It is due every even numbered year covering activities conducting during the preceeding odd numbered year. It must be submitted by large quantity generators and treatment, storage, and disposal facilities.

Submittal Requirements

  • For 2009, the Hazardous Waste Biennial Report is required from Large Quantity Generators and Treatment, Storage, and Disposal facilities
  • Reports must be postmarked by March 1, 2010.
  • Extensions until April 1, 2010 may be requested. Send the request to "info at idem.in.gov" and provide a reason for the request.

Changes to the 2009 Biennial Report

  • GM FORM: A waste minimization code has been added to this form.
  • HANDLER ID FORM: Waste codes have been added to this form. Please enter only the four most commonly used codes used by your facility.
  • SITE ID FORM (in the Easitrak software):
    • Several fields have been added to this form. They are the contact & owner address, short term generator flag, transfer facility flag, & waste received from off site flag.
    • The waste codes are now required to be completed on this form.
    • A section for the 40 CFR Part 262 Subpart K information has been added.
    • An addendum for 40 CFR 261 Hazardous Secondary Material Activity(HSM) has been added.

Note: The Subpart K and HSM rules have not yet been adopted by Indiana and are NOT required to be completed at this time.

Indiana Hazardous Waste Handler ID Form

The Handler ID Form is required to be submitted by all LQGs, SQGs, TSDs, and any facility that recieved a reminder notice in December. If you did not receive an Indiana Hazardous Handler Identification form, request one from the IDEM Handler ID Form page.

  • The ID form should be evaluated for accuracy, corrections made if needed, signed and submitted with the rest of the report if filing in hard copy.
  • If filing electronically the ID form should be sent after the electronic report is submitted.
  • The ID form is NOT the entire report.

Electronic Reporting

  • This is the preferred method of submittal. The American Resource Management (ARM) Easitrak software is available online.
  • Please read the Indiana instruction booklet carefully in addition to the Easitrak instructions.
  • If you don’t do that at least review the ARM Easitrak Quick Notes.
  • The Handler Identification form MUST be submitted in hard copy even if you file electronically. Do not send the form until after you submit the electronic report.
  • This year in addition to the Indiana Handler Identification form, we will also need the Easitrak "Declaration of Electronic Submittal" form signed and sent in hard copy. This is a requirement in order for Indiana to remain compliant with the U.S EPA CROMERR standards.
  • The Easitrak Site ID form must be filled out on line correctly, but need not be printed and sent.
  • If you did not receive an Indiana Hazardous Handler Identification form, request one from the IDEM Handler ID Form page.
  • Handlers who have their own data systems and can create flat files may submit files in the specified format. Contact IDEM (info at idem.in.gov) for a copy of the flat file specifications.

Biennial Report Forms and Instructions

If you are unable to use the Easitrak web version of the Biennial report you may file on paper. The forms are available in both PDF and DOC fillable formats.

Fillable Forms

  • MS Word fillable forms allow you to enter your data into a form on the computer.
  • Open the form and save it under a new name.
  • Each page must be saved under a different name, i.e. Company_name_pg1, company_name_pg2, etc.
  • Use the tab key to move from field to field
  • The forms can then be printed and mailed or e-mailed to us.
