The AMR consists of two parts: the Handler ID Form and the Off-Site Shipments Form (OS Form). The Handler ID Form must always be submitted in hard copy with an original signature. The OS Form may be submitted electronically (preferred) or in hard copy.
If you are normally a Small Quantity Generator, but happened to fall below the minimum for that status, enter the status of Conditionally Exempt Generator. Then, if you expect to return to being a Small Quantity Generator for the next reporting year, enter that status in the field for the next year. Please refer to the full set of instructions to determine your generator status.
Only ONE waste per OS Form is permitted. Each separate waste must be reported on its own OS Form.
Give a short narrative description of the waste. This can be found in Box 9b of the Uniform Hazardous Waste Manifest.
Enter the 4-character waste code(s) that belong to the waste reported. Waste codes begin with a single letter (P, U, K, F, or D) and are followed by 3 numbers. These can be found in Boxes 13 of the Uniform Hazardous Waste Manifest. Lab packs have a waste code of LABP. If the waste code is NHAZ or indicates NONHAZ, the waste is non-hazardous and not required to be reported.
Enter the 12-character RCRA Identification Number of the off-site Treatment, Storage, or Disposal (TSD) Facility to which the waste was shipped. This can be found in Box 8 of the Uniform Hazardous Waste Manifest. If the waste was shipped to more than one TSD facility, make a separate entry for each one.
Include the city and state where the TSD facility is located, along with the name, found in Box 8 of the Uniform Hazardous Waste Manifest.
Enter the total amount of waste shipped to the TSD facility. Justify the amount to the right, and notice the decimal place is already included.
Check the appropriate box for the quantity reported. The only options are pounds, kilograms, tons, and short tons. Volume measurements, such as gallons or cubic yards, are unacceptable, and must be converted to a weight or mass by using the density of the waste. The weight can also be obtained from the TSD facility.
Enter the code that describes what the listed TSD facility actually did with the waste. Do not list the ultimate disposition of the waste. Management codes are 4 characters and begin with the letter H, followed by three numbers. The code can be found in box 19 of the Uniform Hazardous Waste Manifest.
Enter the total number of shipments sent to each TSD facility listed.
Enter the correct name and RCRA Identification Number for each transporter used to ship the reported waste. This is found in Boxes 6 & 7 of the Uniform Hazardous Waste Manifest.
