By Mail       

1. Sign and print your name and address on the back of your ticket.
2. Print a copy of the WINNER CLAIM FORM (one form per ticket)
   • Leave lines 1-6 blank
   • Complete lines 7-21
   • Line 22- please initial this line indicating that you understand the Hoosier Lottery’s tax withholding policy and are aware the Lottery suggests talking with a tax professional.
   • Sign and date the form on the lines indicated.
     Please note: the claim form must be filled out in its entirety or your claim may be returned to you.

     
     

   *	If more than one person is claiming the prize, you will also need to complete IRS FORM 5754 and include a copy of each person's government issued photo id, id (if applicable - see above), telephone number and county number.

   
   
3. For prize claims of $600 or more - include a photocopy of your valid Photo Identification (e.g.- drivers license, military ID, etc.)
4. We suggest you make a photocopy, for your records, of the WINNER CLAIM FORM and the ticket. Be sure to copy both the front and the back of the ticket.  If applicable, also photocopy IRS FORM 5754.
5. Staple your original ticket to the completed claim form.
6. Mail the following items to the address below:
   • WINNER CLAIM FORM with attached ticket
   • Photocopy of valid Photo ID (if applicable - see above)
   • IRS FORM 5754 (if applicable - see above*)
