Care Management Program for Medicaid Select & Traditional Medicaid Populations

The State of Indiana recognizes the many benefits of having coordinated care for all persons in Medicaid. As such, the State has decided to implement a care management program, called Care Select, for the population currently under Medicaid Select and for many of those in our current traditional Medicaid fee-for-service program..

The Administration strives to create a program that will:

  • More effectively tailor benefits to the aged, blind and disabled (ABD) population's needs by using evidence-based medicine to manage services by duration, scope and severity,
  • Improve the quality of care and health outcomes for the ABD population, and
  • Control the growth of health care costs for the ABD population.

This website is dedicated to offering current information on the progress of the project, community presentations, who serves on the Care Select advisory committee, and how you can offer your feedback and thoughts to FSSA.

What is the Care Select Program?

Care Select is a care management program created by the Indiana Family and Social Services Administration (FSSA) to serve Hoosiers. Care Select will:

  • Tailor benefits to people more effectively
  • Improve the quality of care and health outcomes
  • Control the growth of health care costs
  • Provide a more holistic approach to member’s health needs

Who is covered by the Care Select Program?

The following Indiana Health Coverage Plan (IHCP) members will be covered by the Care Select Program:

  • Aged
  • Blind
  • Physically and mentally disabled
  • Members receiving adoption assistance
  • Members in the Waiver Program
  • M.E.D. Works participants

Who is not covered by the Care Select program?

The following Indiana Health Coverage Plan (IHCP) members will not be covered by the Care Select Program:

  • Members on spend-down
  • Members dually eligible for Medicare and Medicaid
  • Individuals with Qualified Medicare Beneficiaries (QMB) or Special Low Income Medicare Beneficiaries (SLMB) only (not in combination with another aid category)
  • Persons in nursing homes, intermediate care facilities for the mentally retarded (ICF-MR) and State-Operated facilities
  • Members in the Hospice Program
  • Undocumented aliens
  • AID to Recipients in County Homes (ARCH) members
  • Members enrolled in the 590 Program
  • Members enrolled in the Breast and Cervical Cancer Treatment Services Program

What is included in the Care Select Program?

Care Coordination:
Individualized services as well as assistance in gaining access to needed medical, social, educational, and other services.

Disease Management:
Population-based, as well as targeting specific diseases. The Chronic Disease Management Program includes diabetes, asthma, congestive heart failure, and hypertension.

Utilization Management:
Appropriate use of facilities, services and pharmacy.

Who will provide Care Select Services?

Care Select services will be provided by FSSA’s two Care Management Organizations (CMOs):

  • Advantage Health Solutions
  • MDwise, Inc.

Care Select Implementation Schedule

November 1, 2007,

  • Central Region: Boone, Hamilton, Madison, Putnam, Hendricks, Marion, Hancock, Morgan, Johnson, Shelby, and Rush

March 1, 2008,

  • Northwest: Lake, Porter, LaPorte, Newton, and Jasper
  • Northeast: Lagrange, Steuben, Kosciusko, Noble, Dekalb, Whitley, Allen, Miami, Wabash, Huntington, Wells, and Adams
  • North Central: St. Joseph, Elkhart, Starke, Marshall, Pulaski, and Fulton
  • East Central: Benton, White, Warren, Tippecanoe, Carroll, Fountain, Clinton, Montgomery, Vermillion, Parke, Vigo, Clay, and Sullivan
  • West Central: Cass, Howard, Tipton, Grant, Blackford, Jay, Delaware, Randolph, Henry, Wayne, Fayette, and Union
  • Southeast: Bartholomew, Decatur, Franklin, Jackson, Jennings, Ripley, Dearborn, Ohio, Switzerland, Washington, Scott, Jefferson, Crawford, Harrison, Floyd, and Clark
  • Southwest: Owen, Green, Monroe, Brown, Knox, Daviess, Martin, Lawrence, Gibson, Pike, Dubois, Orange, Posey, Vanderburgh, Warrick, Spencer, and Perry
