Program Information for Providers

Hoosier Healthwise is made up of two delivery systems:

  1. Primary Care Case Management (PCCM). This plan network is referred to as PrimeStep.
  2. Risk-Based Managed Care (RBMC). This delivery system is made up of Managed Care Organizations (MCO), which are state-contracted HMOs.

In the Hoosier Healthwise program, member's are encouraged to choose a doctor in order to promote a doctor-patient relationship and to provide a medical home for the member's health records. Eligible members residing in any Indiana county will be required to enroll with a Primary Medical Provider (PMP).

In order to encourage the medical home concept, the member chooses a doctor and not a plan. Even if the doctor switches health plans, the member stays with the doctor and moves to the new plan unless the member elects to change to another doctor.
