Address and phone number changes - If you have a change of address or phone number please contact the Hoosier Healthwise Helpline or the place where you applied as soon as possible with the correct information. Important health information is mailed to Hoosier Healthwise members during the year, including health reminders, newsletters, and renewal information. Changing your child's doctor - You may change your child's doctor (PMP) once a year without a reason. If you are not happy with the service or care you are getting from your child's doctor, talk to him or her about it. If you still are not happy, call your Benefit Advocate or the Hoosier Healthwise Helpline at 1-800-889-9949. He or she will help you pick a new doctor if the problem cannot be resolved. He or she may not always be able to change the PMP for you.
Complaints and Grievances - If you have a problem that you cannot solve on your own, there are processes and procedures available to help you solve your problem. You must alert the program to the grievance or problem within 90 days of finding the problem. Call the Hoosier Healthwise Helpline at 1-800-889-9949 for help to file a complaint or grievance.
Renewal - Your membership in the Hoosier Healthwise program must be renewed regularly. You will receive a letter in the mail when it is time to renew. If you do not renew your Hoosier Healthwise membership your services may end. If you renew on time, then your Hoosier Healthwise program services can continue without interruption. If you have a change of address or phone number please contact the Hoosier Healthwise Helpline or the place you applied as soon as possible with the correct information.
