The State of Indiana is submitting this project to maximize consumer choice and self-determination. The Family and Social Services Administration (FSSA) will serve as the lead agency. The $1,147,500 in federal funds will be used to implement the following changes:
- Focus on systems change. The proposal will create an enduring infrastructure to support consumer-directed personal assistance services.
- Direct services. No more than 20 percent of the total grant dollars will be used to directly fund consumer directed attendant care.
- Collaborators and partnerships. FSSA, as the lead agency, will work closely with other state agencies, the CHOICE Board, the Arc of Indiana, the Indiana Area Agencies on Aging, Independent Living Centers, local advocates and other organizations to institute information, outreach, and establish statewide policies.
- Enhanced training. State staff and local providers will participate in enhanced training to promote consumer-directed care, person-centered planning, quality assurance and fiscal agent coordination.
- Outcome-Based Reporting. Evaluations will be conducted of the timeliness, adequacy and quality of the services provided, as well as the outcomes and impact of self directed personal attendant care services care on quality of life measures.
