Overview of WorkEthic Certification Program that describes eligibility, certification levels, and the ten Criteria
Employer Forms
Employer Intent to Participate Form
Employers complete and return the form to sign up as a participant and pledge to recognize the WorkEthic Certificate. The form collects information on the employer's willingness to offer internships and additional programs.
School Forms
Participating School Fact Sheet
Schools complete and return the form to sign up as a participating school. The form requires contact information of the principal and Designated School Liason, number of eligible students, and important school dates.
Student Intent to Participate Form
Students and parents must complete and return the form to participate in the program. Forms are returned to the Designated School Liason.
Community Service Documentation Form
Students complete the form by documenting community service projects, hours volunteered, dates of service, and project supervisor. Students earn one point by completing six hours of community service and two points for 12 hours of service.
Internship Documentation Form
Students complete the form by documenting the employer, employment dates, and hours worked.
Student Evaluation Form
The Designated School Liaison and teachers complete the evaluation form. The points are tallied and the total points earned determine whether the student receives a blue or gold certificate.
