1099 Help:

If you moved and did not report your change of address to DWD by 12/31/2007, effective 2/5/2008, a copy of your 1099G can be obtained from a WorkOne office.
 
Duplicate forms may also be requested by clicking on the 1099HELP@dwd.in.gov.  You may also use this e-mail if you have a discrepancy on your 1099G.  Please include the following information:

  • Year requested
  • Social security number
  • Name
  • Current complete address
  • Current telephone number
