Individual Taxpayer Questions

  1. What if I have not yet filed my individual tax return yet and all my paperwork has been destroyed or lost?
    You will need to contact the organizations or employers who provided them to you initially. They should have copies. For help, contact 317-233-5212.
  1. What if I live in an area affected by severe weather and I’m expecting a refund from the state – but, the refund hasn’t arrived yet?
    The Indiana Department of Revenue (DOR) will work with you to discover the status of your refund and, if due, will work with you to get that refund to you. Contact us at 317-233-5212.
  1. What if I live in an area affected by severe weather and my refund check was destroyed before I could cash it?
    Customers can request a replacement check by completing an Affidavit for Lost or Not Received Warrant form. Contact 317-233-5212 for assistance with obtaining this form.

Business Taxpayer Questions

  1. What if my business is in an area affected by severe weather, and I’ve lost my sales-tax records?
    DOR can assist you with those records call. 317-233-5212. Make certain you verify your current or temporary mailing address when you call.
  1. What if my business is in an area affected by severe weather, and I’ve lost my trust-tax coupons?
    DOR can send you replacement trust-tax coupons through the mail, or at one of our district-office locations in person. Select the closest District Office (Columbus or Clarkesville).
  1. What if my business is in an area affected by severe weather, and I’ve lost my payment-plan coupons?
    Customers can get replacement payment-plan coupons through the mail, or by visiting any district-office location to receive them in person.
  1. What if my business is in an area affected by severe weather, and I’ve lost my RRMC (registered retail merchant certificate)?
    Customers in an area affected by severe weather can replace their active RRMC through the mail, or by visiting any district-office location to receive the RRMC in person.
  1. What if I live in an area affected by severe weather and I am unable to make payment on my bill at this time?
    DOR understands the taxpayer’s situation. DOR will make every effort to work with you in coming to a reasonable agreement for payment on a case by case basis.  Contact the DOR at 317-233-5212 to discuss with a DOR representative.

    If you need a copy of your most recent return, contact DOR at 317-233-5212 and explain your needs. We will recreate a copy of your return from our records and send it to you.
  1. What if I am a farmer or self-employed, and I live in an area affected by severe weather – and I need a copy of my state return to verify my income for the Department of Workforce Development?
