Carriers who cross state lines will need to apply for a Motor Carrier number by using form

OP-1(P) for Passenger Authority and form OP-1 if transporting property for hire. These carriers will also need a USDOT number, which can be obtained using form MCS-150A. These forms can be found at the Federal Motor Carrier Safety Administration Web site at http://www.fmcsa.dot.gov/.gov and will take 45-60 days to process.

Carriers who transport 9 or more passengers will also need to apply for a Fuel Permit by completing Form MCFT-1. There is a $25 application fee for this. Carriers also will need to report on a quarterly basis their miles traveled and fuel purchased.
