Fatalities

Map of Occupational Fatalities though May 2008

  • RED - IOSHA inspected fatality
  • GREEN - Occupational fatality outside of IOSHA jurisdiction
  • BLUE - IOSHA inspected catastrophe (High profile incident or at least 3 employees hospitalized)

Employers are required to report an employee death within 8 hours of the incident to the Department of Labor. IOSHA (Indiana Occupational Safety and Health) inspects fatalities, within their jurisdiction, where an employee/employer relationship exists. Occupational fatalities which are usually not followed by an IOSHA inspection include:

  • Deaths from natural causes such as heart attacks or strokes (death certificate is tracked by IOSHA)
  • Fatalities occuring on federal land
  • Homicides
  • Fatalities occuring on a family farm
  • Vehicle highway accidents
  • Suicides
  • Self-employed individuals

Census of Fatal Occupational Injuries

In partnership with the Bureau of Labor Statistics the Quality, Metrics, and Statistics Division completes an annual census of all fatal occupational injuries (CFOI). The data from this survey can be searched or viewed through the following links:
