Welcome to the Department of Natural Resources Division of Entomology and Plant Pathology survey report form for the Forest Pest Outreach and Survey Project (FPOSP) conducted in cooperation with the USDA Animal Plant Health Inspection Service (APHIS). Thank you for participating in the survey to detect the presence of Emerald Ash borer (EAB), Asian Longhorned Beetle (ALB) and Thousand Canker Disease of Black Walnut TCD).

For each paper survey form that you completed in the field, complete one electronic survey report form and email it with any photographs (in jpg) to DEPP@dnr.in.gov  Please complete an electronic survey report form for each tree or group of trees that serve as survey location whether the tree(s) have symptoms or do not have symptoms. Completed forms will be reviewed by DEPP staff who will contact you to arrange for any follow-up visit to the survey site to examine the tree(s) you report.
