The following two forms are also used for Kid's First Trust Fund. The third form is used for Community Based Child Abuse and Prevention Program. These forms can be completed online and then printed, but cannot be saved.

Funds under this program may be used for developing, operating, expanding, and enhancing statewide networks of community-based, prevention-focused, family resource and support programs that:

  1. Prevent child abuse and neglect;
  2. Decrease the risk of homelessness;
  3. Provide respite care services;
  4. Improve families’ access to formal and informal community resources that prevent child abuse and neglect, and prevent homelessness;
  5. Provide or arranges for the provision of family resource and support services;
  6. Provide family resource and support outreach services;
