Disability parking placards and disability license plates allow the holders to park in areas designated for disabled drivers.

Disability Parking Placard

A disability parking placard is available to an Indiana resident of any age who meets one of the following requirements:

  • has a temporary or permanent physical disability that requires the use of a wheelchair, a walker, braces, or crutches;
  • has temporarily or permanently lost the use of one or both legs;
  • has restricted mobility, either temporarily or permanently, due to a pulmonary or cardiovascular disability, arthritic condition, or orthopedic or neurological impairment; or
  • is permanently blind or visually impaired.

Disability parking placards are also available to companies or associations which transport or provide facilities for persons with physical disabilities.

Length of validity

A disability parking placard issued to an individual with a permanent disability will expire on the recipient's fourth birthday following the date of the placard's issuance.

A placard issued to an individual with a temporary disability is valid for six months from the date that it is issued, or until a date indicated by the physician certifying the application.

When the placard expires, you must apply again for another placard.

Getting A Disability Parking Placard

In order to receive a disability parking placard, you must complete an Application For Disability Parking Placard Or Disability Plate - State Form 42070. The application must be certified by any licensed physician.

Return the completed form to any Indiana license branch or mail application and payment, if applicable to the address on the form to receive your disability parking placard.

Disability License Plate

The cost of the disability license plate includes only vehicle registration fees and taxes.

A disability license plate or disability parking placard allows the holder to park in areas designated for disabled drivers.

Eligibility

Disability license plates are available to Hoosiers who:

  • Have a permanent disability that requires the use of a wheelchair, walker, braces or crutches;
  • Or have permanently lost the use of one or both legs;
  • Or experience restricted mobility due to certain medical conditions;
  • Or are permanently blind or visually impaired.

In order to receive a disability license plate, you must complete an Application For Disability Parking Placard Or Disability Plate - State Form 42070. The application must be certified by an Indiana physician.

Availability

This plate is available online through myBMV, at license branches, and at other customer service locations.

Vehicle Types

This plate may be displayed on passenger cars, motorcycles, trucks up to 11,000 pounds and RVs.
