Driver Or Vehicle Records Request
To request a person's driving record you must complete and submit Request for Driving Records - State Form 53789.
To request a vehicle or watercraft registration history, you must complete and submit Request for Motor Vehicle or Watercraft Records - State Form 46449.
Completed forms may be emailed to Dennis Rosebrough or Graig Lubsen, or faxed to 317-232-8762.
