Filing a Do Not Fax Complaint

We are committed to ensuring Indiana’s Do Not Fax law is enforced. To do this, we need your participation in two very important ways.

First, file a complaint if you receive an unsolicited fax you believe has violated the law. Provide as completely as possible, all the information requested on the complaint form. Items noted with an asterisk (*) are especially important as they indicate the minimum information the office needs to begin an investigation.

Second, your cooperation with our attorneys and investigators will enable us to investigate and resolve your complaint in a timely manner.

We require that you fill out a complaint form and provide us a copy of the unsolicited fax. You can file a complaint by filling out the complaint form, printing it out, signing the second page and mailing to us with a copy of the unsolicited fax.
