Governor's Arts Awards Nomination Form

All contact information provided will be treated as confidential and will only be used for the purposes of contacting prospective nominees and nominators. Direct all questions to Rex Van Zant at rvanzant@iac.IN.gov or 317-232-1273.

Only this form or the mail-in form available from the IAC may be used for nominations. No other materials should be submitted. All valid nominations not selected for an award will be eligible for future consideration. Nominees must be willing and available to attend the awards ceremony. Recipients should agree to assist with media requests related to their award. If an individual is being nominated posthumously, provide name and contact information for a relative who will be able to accept the award.

Nomination Narrative

This should summarize the major work, accomplishments and contributions of the nominee. List any significant awards, honors and recognition that support the accomplishments and contributions. A resume, general biography, or press kit will NOT be accepted as a substitution for the required nomination narrative.

The narrative should address as many of the following criteria as apply to your nominee:

  • The nominee's accomplishments or contributions to the arts in Indiana
  • The quality of work or contribution to the arts
  • The nominee's demonstration of continuing history of outstanding work or contribution to the arts
  • The impact of the nominee's work or contribution to the arts in his or her community, region, the state of Indiana, or elsewhere
  • The impact the nominee's work or contribution has to inspire and encourage others to follow the example of the nominee
  • How would selecting this nominee result in a significant positive reflection on the arts in Indiana?

After review of the nomination information, the IAC may request additional supporting information. Do not send additional information or work samples until such time they are requested. Recipients and their nominators will be notified after the selection process is completed and the list of nominees has been approved by the Governor.
