You need one document to prove your identity when you apply for a new driver's license, permit, or identification card.

Non-U.S. citizens who have received a SecureID driver's license or identification card, after submitting one of
the documents in this list, may use their Indiana driver's license or identification card to prove their identity.
Proving A Change In Name, Date of Birth, Or Gender
If your current legal name is different from that on the identity document you selected above, you must provide
legal proof clearly showing how your name changed from the one on your identity document to your current legal name.
The proof must be in the form of original or certified documents, such as a marriage certificate, divorce decree, or court order.
In some cases, such as multiple name changes, you may need to provide several documents showing the connection between
your current legal name and the one on your identity document.
