DEADLINE EXTENDED TO REQUEST REPLACEMENT FOOD STAMPS IN 11 INDIANA COUNTIES DAMAGED BY SEVERE STORMS AND TORNADOS

(INDIANAPOLIS) – Indiana Family and Social Services Administration (FSSA) has received approval from the USDA Food & Nutrition Service (FNS) to extend the time period for reporting food loss caused by severe storms and tornados March 2, 2012, in 11 southern Indiana counties. The new deadline for Hoosier families to apply for replacement food stamp benefits is March 22.

This benefit replacement is limited to clients currently receiving food stamp benefits (Supplemental Nutrition Assistance Program – SNAP), and who reside in counties designated as disaster emergencies. Gov. Daniels declared the following as disaster emergency counties: Clark, Gibson, Harrison, Jefferson, Posey, Ripley, Scott, Shelby, Vanderburgh, Warrick and Washington.

To qualify for a replacement of benefits, you must report this loss by March 22, and sign an affidavit for replacement of the food stamp benefits. Division of Family Resources (DFR) staff may request proof of loss such as a statement from the utility company, insurance report or FEMA application.

There are two locations for replacing food stamp benefits:

  1. Local DFR office in your county of residence. (Addresses listed below)
  2. One Stop Shop for Tornado Disaster Relief. Indiana’s central resource center, located on the first floor of Ogle Hall, Ivy Tech campus in Sellersburg, near I-65 and Highway 311 (I-65 Exit 9), open 8 a.m. to 8 p.m., seven days a week for at least 10 days

Additional assistance may be available once the state has completed its damage assessment and has requested federal assistance.

If a Hoosier Works EBT card has been lost as a result of the March 2 tornados and storms, a replacement card can be requested. Contact Hoosier Works EBT Customer Service TOLL FREE: 877.768.5098.

Families who need assistance with their Child Care Development Fund (CCDF) benefits should contact: River Valley Resources TOLL FREE: 866.652.7027, or the Bureau of Child Care TOLL FREE:  877.511.1144 (press 3 when prompted).

FSSA’s Mental Health Crisis Counseling Line is available TOLL FREE: 866.679.4631. This crisis line provides mental health and recovery counseling for those impacted by the storms.

General information about Food Stamp (SNAP), Medicaid and TANF benefits is available TOLL FREE: 800.403.0864.
